NOTICE: If you received a NOTICE OF DATA BREACH letter from Personic Management Company LLC (“Personic”), contact Emery | Reddy, PC at 916.995.5968 for a Free Case Review.

On or about September 1, 2025, Personic Management Company LLC became aware of unauthorized activity involving a third-party software platform used to process patient information. The company immediately secured the environment and engaged cybersecurity experts to investigate the incident. The investigation determined that an unauthorized actor accessed the platform on August 29, 2025, and acquired certain data. On October 13, 2025, the review determined that some of your PHI was subject to unauthorized access and acquisition.

While there is no evidence of identity theft or fraud, the review concluded that some protected health information (PHI) may have been involved. Personic is notifying affected individuals out of an abundance of caution and offering 24 months of complimentary credit monitoring and identity protection services through Cyberscout.

Personic Management Company LLC is a healthcare organization that provides in-home mobile wound care services through its brand, Personic Health, and also acts as a Managed Services Organization for physician practices.

Following the breach, Personic implemented enhanced security measures and continues to monitor its systems.

If you received a Data Breach notification letter from Personic, it confirms that your information was potentially impacted.
