NOTICE: If you received a NOTICE OF DATA BREACH letter from Amazing Charts, contact Emery | Reddy, PC at 916.995.5968 for a Free Case Review.

On or about June 19, 2025, CareTracker, Inc., operating as Amazing Charts (“AC”), discovered unusual activity within a network managed by one of its third-party service providers. The company immediately secured the environment and engaged external legal counsel and cybersecurity specialists to investigate the incident. The investigation determined that unauthorized access occurred between June 15, 2025, and June 19, 2025.

While there is no evidence of identity theft or fraud, the review concluded that some personal information may have been involved. Amazing Charts is notifying affected individuals out of an abundance of caution and offering 12 months of complimentary credit monitoring through Experian IdentityWorks.

Amazing Charts is a healthcare software suite that includes an electronic health record (EHR) and practice management tools designed to help independent medical practices streamline administrative tasks, improve billing, and manage patient records. It is utilized by healthcare providers such as Genaro C. Fernandez, MD.

Following the breach, Amazing Charts implemented enhanced security measures and continues to monitor its systems.

If you received a Data Breach notification letter from Amazing Charts, it confirms that your information was potentially impacted.

Your Personally Identifiable Information (PII) includes details that can be used to identify you.
